As part of our continued effort to promote an active membership and increase communication, SSYHA has created additional methods of providing feedback to it's leadership. Please use the links below to access the appropriate form to communicate any feedback you wish to share, or to report an incident that needs to be brought to the attention of the club.

The 'Feedback Form' can be used to share any feedback with the club, positive or negative, and can be anonymous.

The 'Reporting Form' can be used to document minor incidents concerning a club member, or bring more serious incidents to the attention of the leadership. These reports are confidential, but not anonymous.
